is offered through Delta Dental of Ohio. The plan provides full coverage for diagnostic and preventive care with additional coverage for other dental services. Orthodontia is covered for both adults and children with a lifetime maximum benefit of $1,200 per covered person.

Â鶹ÊÓƵÈë¿Ú offers two administered by VSP. Both plans provide discounts on routine eye exams, frames, lenses and contact lenses.

Â鶹ÊÓƵÈë¿Ú offers a to allow you to pay for eligible out-of-pocket healthcare and dependent care-related expenses with pre-tax dollars.

Â鶹ÊÓƵÈë¿Ú offers through Securian/Minnesota Life. Coverage options include the following:

• Basic Life: covers full-time employees at 2x base pay, not to exceed $150,000
• Supplemental Life: $10,000 increments up to $500,000
• Spouse Life: $10,000 increments up to $200,000
• Dependent Child Life: $10,000 per covered child

is provided for full-time employees through Unum. The plan provides a benefit equal to 60% of your base pay, not to exceed $5,000 per month.
